Who is eligible for the HEART Rx Initiative?

If you are paying in cash and do not have insurance coverage or benefits for fertility medications, then you are automatically eligible for the HEART Rx Initiative.

Q.

After I've been prescribed MENOPUR^® and/or BRAVELLE^®, how do I enroll in the program?

A.

It’s easy. Just bring your prescription for MENOPUR^® and/or BRAVELLE^® to a participating specialty pharmacy. See the link below for a printable pharmacy list.

What happens next?

A.

You simply pay a $10 annual enrollment fee. In turn, the pharmacist will assign you a membership number, and your savings can begin immediately—no waiting, no rebates. Be sure to write your number on the membership card that comes with the brochure and keep it handy.

Q.

What happens if I lose my membership card?

A.

No worries. Once you're entered in the system, the pharmacist can easily look up your number. However, having the card does save time, and it has a place for keeping track of doctor appointments or other important dates. So we encourage you to tuck it in your wallet or another convenient place.

How much will I save?

A.

It all depends upon how many vials you’ve been prescribed. You’ll receive savings on 100 vials of each medication, for a total of up to $2100:

• Save $18 on each vial of BRAVELLE^® prescribed = $1800 savings
• Save $3 on each vial of MENOPUR^® prescribed = $300 savings

For example, your first cycle may require a prescription of 40 vials each (a savings of $720 for BRAVELLE^® and $120 for MENOPUR^®). If you require a second cycle of 40 vials each, and it is within a year of your enrollment date, you will still be covered by your annual membership for a total annual savings of $1680.

Are refills covered?

You are covered for up to 100 vials of each medication.

Q.

What if I need more than 100 vials of each medication?

A.

If your doctor prescribes 100 vials of MENOPUR^® and/or BRAVELLE^® and you need more vials, your doctor can write an additional prescription, and if within a year of your enrollment date, you will receive the discount on the vials covered by your new prescription.

Q.

What if I exceed the enrollment year? Can I renew my membership?

A.

If you exceed the enrollment year, you simply pay another enrollment fee of $10 and continue to receive the discount on an additional 100 vials each of MENOPUR^® and/or BRAVELLE^® per prescription.

Q.

What if I move or travel?

A.

The HEART Rx Initiative is a national program; if you are anywhere in the United States (including Hawaii and Alaska), and go to a participating pharmacy, your membership will be valid and should continue.
