Yes, cash-paying patients are eligible for the h.e.a.r.t.+ program.

If I am a cash-paying patient, which drugs do I save on with the h.e.a.r.t.+ card?

The h.e.a.r.t.+ card is valid for significant savings on BRAVELLE^®, MENOPUR^®, NOVAREL^®, and ENDOMETRIN^®.

How much will I save if I'm paying cash for my prescription(s)?

If you are paying cash, you will receive significant savings on BRAVELLE^®, MENOPUR^®, ENDOMETRIN^®, and NOVAREL^®. Speak with your participating h.e.a.r.t.+ pharmacy for pricing^‡.

After BRAVELLE^®, MENOPUR^®, ENDOMETRIN^®, and/or NOVAREL^® have been prescribed, how do I enroll in the program?

It's easy. Just ask your doctor or nurse for a h.e.a.r.t.+ card. Bring your prescription and the h.e.a.r.t.+ card to a participating pharmacy, pay an activation fee of $10, and start saving instantly on your prescriptions for BRAVELLE^®, MENOPUR^®, ENDOMETRIN^®, and/or NOVAREL^®.

What happens after I enroll?

Start saving on your prescription(s)—no waiting, no rebates.

Can the h.e.a.r.t+ card be used at any pharmacy?

No. The h.e.a.r.t.+ card is valid for use only at participating pharmacies.

Can I use more than one card to obtain bigger savings?

No. There is a limit of one card per patient.

How long is my h.e.a.r.t.+ card valid for?

This h.e.a.r.t.+ card is valid for one calendar year from the date of activation.

What happens if I lose my h.e.a.r.t.+ card?

No worries. Once you're entered into the system, the pharmacist can easily look up your unique ID number. However, having the card does save time, so we encourage you to tuck it in your wallet or another convenient place.

What if I move or travel?

The h.e.a.r.t.+ program is a nationwide program; if you are anywhere in the United States and visit a participating pharmacy, your membership will be valid.

I am an insured patient. Am I eligible for the h.e.a.r.t.+ savings program?

If I have coinsurance, am I still eligible to save with the h.e.a.r.t.+ card?

Yes, you are eligible for savings. Those savings, however, must be used against your copay.^II

If I am an insured patient, which drugs do I save on with the h.e.a.r.t.+ card?

The h.e.a.r.t.+ card is valid for additional savings on BRAVELLE^® only.

How much will I save if I have insurance?

If you have insurance, you will save up to $500 on each prescription for BRAVELLE^®.

If I am an insured patient, how do I qualify for the h.e.a.r.t.+ program?

To qualify for this offer, the first prescription for BRAVELLE^® must have a minimum of ten (10) 75 IU vials of BRAVELLE^®. There is no quantity minimum on refills.

After BRAVELLE^® has been prescribed, how do I enroll in the program?

It's easy. Just ask your doctor or nurse for a h.e.a.r.t.+ card. Bring your prescription for BRAVELLE^® and the h.e.a.r.t.+ card to a participating pharmacy and begin saving immediately.
